The failure mode of agent systems is not hallucination. It is an agent with the authority to take an irreversible action and no reason to hesitate.
We design the tool surface before the agent. Each tool has a contract: what it does, what it costs, whether it can be undone, and who is accountable if it goes wrong. Irreversible actions require an approval step, always.
Spend caps and rate limits are enforced at the infrastructure layer rather than by instruction. An agent cannot be prompted out of a hard limit.
The result is a system whose worst case is understood. That is the condition for putting it anywhere near a customer.
If you cannot measure the change, you cannot defend the system that caused it.
